Important Notice

  1. The registration fee will apply to all Leo attendees .
  2. Name tags will be distributed with your Forum registration materials and need to be worn in order to gain admission to all meals, activities and seminars.
  3. Payment for registration fee, hotel, and all meal costs must be included with your registration form.
  4. Cancellations must be directed to info@usallf.org or 646-481-3238 with detailed information. For Example: Applicant Name, District, Club Name, and phone number
  5. Refunds will be sent following the convention. Refund requests deadline is: July 1st, 2018.
